Notice of Claims Procedure for MDNY

Historical Information:

MDNY Healthcare Inc.(“MDNY”) was a New York corporation licensed to operate as a health maintenance organization pursuant to Article 44 of the New York Public Health Law.

By unanimous consent of the board of directors, MDNY consented to entry of an order of liquidation pursuant to Article 74 of Insurance Law. Notices were sent to members and plan providers terminating coverage and contracts as of May 31, 2008.

An Order of Liquidation (“Order”) signed by Justice Alice Schlesinger of the Supreme Court of the State of New York, County of New York, was filed on July 31, 2008.
